Rotation Information
TYSB Clinic
Evaluation, diagnosis, treatment of child and adolescent patients with psychiatric illness. Prescribing of psychotropic medications, providing psychotherapy to patients and families. They will also participate in coordinating care with other therapists and community care providers. At minimum residents have a 1:1 ratio of faculty: resident for supervision. All supervising faculty are board certified. This is a maximum of 2:1 supervision by faculty. Residents will be seeing 2-4 patients per half day depending on the ratio of new evaluations to follow up evaluations. This site provides residents an opportunity to see child and adolescent patients in the outpatient setting, whereas their other Child and Adolescent experience is exclusively inpatient. Patients at these sites come from a very undeserved and rural community leading to greater pathology, which leads to an unbeatable learning experience for the residents.
Central Star Inpatient
Central Star Psychiatric Health Facility is a 16-bed facility in Fresno that focuses on acute care, stabilization, community transition, and aftercare for youth ages 12-17. The clinical population consists of an ethnically diverse group of children and adolescents referred from throughout the county for acute inpatient psychiatric treatment. The patients have severe emotional and behavioral problems encompassing the entire spectrum of psychiatric diagnoses. The facility is one of the finest in the Central Valley and has won several accolades for its dedication to high quality patient care.
Residents are fully credentialed to provide complete care at the facility and complete 1 month of experience at the facility during their PGY2 year.
Residents may optionally return in their PGY4 year to complete a selective at the facility.
Education activities include daily teaching rounds, daily multidisciplinary treatment plan meetings, and one hour weekly individual supervision. During this rotation, residents will learn to manage adolescents of high acuity. They will learn to mitigate risk of harm to self and others through effective intervention to control target symptoms and behavior using the following modalities: Safe and rational use of psychopharmacologic agents; Ward behavior management; Group therapy with psychiatrically ill adolescents.
Residents will have an average caseload of 6-8 high acuity patients. They will assume primary responsibility for their patients, and work collaboratively with the team of psychologists and social workers. In addition, residents will be exposed to different forms of psychotherapy including individual, group, and family therapy through direct collaboration with non-physician staff. Residents primarily work with the site Director Dr. Sukhjit Brar, a board-certified child psychiatrist who has a palpable passion for resident education and they will also work with fellows from the Child and Adolescent Fellowship Program.
